2025 Summer Food Service Program Multiple Day Meal Pickups
This program starts Monday, June 2 and goes until July 25.
*One signup form per child must be completed.
*Any child age 1 through 18 qualifies regardless of income.
*Enrollment in Johnson County Schools or residency in Johnson County is NOT necessary to participate in the program.
*Each child signed up will be provided 7 breakfasts and 7 lunches for pickup from your chosen location.
*Buses will deliver once per week to the location you choose. Your child's meals must be picked up from that location at its designated time to receive their meals.
*If a change in pickup location is needed, please complete this form again with the updated location.
*The child does not have to be present for the meal pickups; however, if a child is not signed up for the program, the child must be present at their first pickup to receive their meals.
